A 50 year old man presents to ED complaining of a hoarse voice. Imaging shows an aneurysm of the aortic arch. Which of the following structures is likely being compressed:

Answer:

The left recurrent laryngeal nerve passes below and behind the aortic arch before ascending towards the tracheo-oesophageal groove medial to the arch of the aorta. Damage to the recurrent laryngeal nerve will result in a hoarse voice.

The pulmonary trunk and the ascending aorta are contained within the pericardial sac, lined by serous pericardium and within a common sheath.

Ascending Aorta

The ascending aorta arises from the aortic orifice at the base of the left ventricle, at the level of the lower edge of the third left costal cartilage, posterior to the left half of the sternum. The ascending aorta ascends, moving slightly forwards and to the right, before continuing as the arch of the aorta posterior to the second right costal cartilage.

Pulmonary Trunk

The pulmonary trunk arises from the conus arteriosus of the right ventricle slightly anterior to the aortic orifice, posterior to the left third costal cartilage. The pulmonary trunk ascends, moving posteriorly and to the left, lying initially anterior and then to the left of the ascending aorta, before bifurcating into the right and left pulmonary arteries posterior to the left second intercostal space.
